This rating is based on a scale of 1-5 stars and reflects the quality of patient care and the experience of the families we serve.
Timely Initiation of Care: 100% (State avg. 94.8%, National avg. 96%)
This rating is based on how quickly we are able to initiate care. We aim to be with your loved one within an hour of your phone call. The higher this rating, the better.
Rehospitalization Rate: 10.4% (State avg. 16.5%, National avg. 15.7%)
This rating is based on how often a patient needs to be re-admitted to the hospital within 60 days after receiving care. The lower this rating, the better.
